Puppy Adoption Agreement
2 Year Health Guarantee
We, the Breeder, guarantee that upon purchase, the puppy is in good health and has been immunized and wormed according to its accompanying health record. The puppy has been vet checked and has a health certificate.
1. To the best of the seller’s knowledge , this puppy is sound and in good health. It is understood that the puppy is to be taken to a licensed veterinarian within 72 hours of receiving the puppy. If the puppy is not examined by a vet of the buyer’s choice within 72 hours, this contract is null and void.
2. If the puppy is found to have a genetic disorder by the veterinarian within the first year of the puppy’s life, we will replace the puppy with the first available puppy of the sa,e value of the breeder’s stock. We provide this warranty for the first 2 years after the date of sale against any debilitating congenital defects. In the event the puppy exhibits symptoms of a suspected debilitating congenital defect the buyer must supply any requested veterinary records to the seller within 7 days of informing the seller of the defect. We have the right to require a second opinion from a veterinarian of the seller’s choice at the seller’s expense. The original puppy must be returned (at the owner’s expense) before a new puppy can be replaced. In the event of death, an autopsy must be returned (at the owner’s expense) by a licensed veterinarian to determine the exact cause of death. This information will then be sent to the breeder’s veterinarian for review.
3. Hypoglycemia, giarddia, coccidia, malnutrition, dehydration, and physical exhaustion are not covered under the terms of this contract. The guarantee against transmissible diseases such as Parvo-virus, for example, is valid for a period of 72 days starting on the day of pick up, since normal incubation for transmissible diseases is 5-7 days. For example: if the first signs of Parvo-Virus were to show on a puppy 12-15 days from pickup, the puppy could not have carried that disease from the breeders premises and incubated it for that before showing signs of the disease.
4. You as the owner are required to: give all immunizations (including rabies), check for intestinal parasites every 3 months until 1 year of age, and start the puppy on heartworm prevention between 4 and 6 months of age.
5. For the purpose of this agreement, once the puppy leaves the seller’s premises, it is under the buyer’s responsibility and the seller is no longer responsible for any veterinary bills. If any treatment is rendered, it is the buyer’s expense.Seller is bound only to the original buyer. No warranties expressed or implied are made under the terms of the contract except as stated above.
Owner’s Responsibility: You, the new owner, agree to take care of this dog; feeding and housing it properly and maintaining its vaccinations and dewormings. You also agree to see to its proper physical and emotional care and welfare. You assume all responsibility for any accidents or injuries caused by the misbehavior of the puppy after it is in your possession. The death, injury or loss of said dog, due to theft, carelessness, recklessness, abuse, neglect, heat stroke, or accident will not entitle you (buyer) to any compensation from us (breeder). This dog is warranted only for the purpose of being a companion animal; not for any other purpose or reason, such as breeding or show, etc.
